All plans include an assigned primary care provider, quarterly preventive reminders, a basic annual preventive lab package per eligible enrolled member, discounts on additional labs, and GoodRx enrollment assistance.

A photo ID, your insurance card if you have one, and a list of the medications you take, including vitamins and anything from the pharmacy.

Your first visit
We take your vitals, go through your history and the physician examines you. If you need lab work or an EKG, it is done here the same day.
Plan on about 45 minutes

Before you leave
The physician goes over the findings and the plan with you, in English or Spanish. Prescriptions are sent to your pharmacy and the follow-up is scheduled at the front desk.

Autonomic nervous system assessment
A non-invasive study that evaluates circulation, nervous system response and early cardiovascular risk. It takes about 15 minutes and requires no preparation.
FibroScan
Liver health without a biopsy
Measures liver stiffness and fat with ultrasound. It is the key tool for tracking fatty liver, a very common and often silent condition.
Electrocardiogram
EKG the same day as your visit
A recording of the heart's electrical activity to evaluate palpitations, chest pain or high blood pressure management, interpreted by the attending physician.

You do not need insurance. We see self-pay patients and the office visit starts at $95, which includes the consultation, the physical exam and your treatment plan. Lab work, studies and vaccines are separate and we quote each one before doing it. We also accept several insurance plans — see the list above or call us.
No. Corazón Salud is a medical services membership program from Eminat Medical Center. It is not health insurance, it is not an insurance plan and it does not replace a health insurance policy. It does not include hospitalizations, major emergencies or specialists outside our agreements. The plans are Essential, Plus and Max.
All plans include a basic annual preventive lab package per eligible enrolled member and annual TM-Flow. Plus and Max include annual EKG; eligible Max members have an annual Pap test or Total PSA benefit. These benefits depend on clinical order, the selected term and membership conditions. Additional labs have member discounts; other tests may have separate charges.
We accept Aetna, Cigna, UnitedHealthcare, UMR, Memorial Healthcare, OSCAR, AmeriHealth, Gold Kidney, Medicare and Medicaid. Plans change often, so call us with your card in hand and we will confirm your specific plan before you come in.

Yes. We perform the complete Form I-693, including required vaccines and lab work, and hand you the sealed envelope ready to file.
Yes. The whole team, from the front desk to the exam room, works in English and Spanish. You don't need to bring an interpreter or request one.
